Host Home Placement
With Host Home services, we provide residential support in a private home, offering an alternative to institutional or group homes. These services include support, supervision, and assistance with activities of daily living. Host families, who are trained and certified on specific needs of the individuals they support, including medication management and behavioral support, provide a home-like environment and support for individuals with disabilities.

Supported Living
Supported Living Services, provides support, supervision, and skill-building to individuals who live in their own homes or community settings, with a focus on supporting their independent living goals.

Custom Support Plan
We provides a variety of different services intended to help people with disabilities participate more fully in their communities and lead more self-determined lives. Please inquire with us if you are looking for a service not listed here, we can often tailor your care plan to your unique needs.

Community Support
Community Based Day Support provides daytime supervision and support for individuals with disabilities to develop and maintain various skills, including self-help, community living, social, and communication skills. It’s designed for individuals who live independently in the community, including those living alone, with roommates, or with a spouse, and helps with activities of daily living.
